Your Visit

Your first visit to our office is important for establishing a strong foundation for your eye care plan. We will take the time to thoroughly understand your medical history and perform comprehensive testing to properly evaluate your eye health. We will then devise a personalized treatment plan, and we will be happy to answer all questions you may have.

We are committed to an individualized care plan that ensures a comprehensive understanding of your eyes, your overall health and wellness, and detection of any early signs of eye disease. We utilize the newest and most innovative equipment to assess your eye health and reduce the risk of any issues.  This allows us to intervene early with treatment plans that minimize any further damage.

Please print and complete these forms prior to your first visit. In order to protect your privacy, do not fax or e-mail these pages; please bring them with you to the office.  Please be sure to bring a valid photo ID and your insurance cards.
